Resignation vs. Termination

This has been dealt with before, but I have a different twist.

I have burnt out on my present position and admittedly, I have not been doing the best work that I could have been doing and management has noticed. I was given 60 days to improve my performance-and was to follow up with my manager w/in 30 days. I immediately started working on my resume and interviwed for a few positions. Within that time period, a family member has become terminally ill and I made the decision to relocate to be closer to that parent. I had discussed my situation with my direct supervisor and advised her that I intend to leave this summer. A week later I met with my supervisor and her boss for our 30 day follow up. I now have an additional thirty days to improve my performance or I will be terminated. It came out in our meeting that my supervisor did not inform her boss of our previous discussion about my plans to leave this summer (who knows why she chose to leave that out). When I informed her boss about my decision, I was told that if I was prepared to give formal written notice, then she would see what she could do about my being paid up until that time-but-she couldn't make any promises.

Well, do I try to bring my work up to standards, which I am not sure if I have the motivation to do at this time, or do I start writing that letter of resignation?
