I would resign and not count on the extra pay. If your heart is not in the work, the work will not get done. I sounds like you have even more on your mind and other things to be working on, like moving. Make the move and start looking for a new job.

I don't know what your previous work history is, but if this is the first time this has happened, it is not a pattern or reflective of your potential. A reasonable employer will take this into account.

Also when looking for a new job, be sure you have clearly worked out in your mind how you will be dealing with your terminally ill relative. How much time will that take? What emotional energy will be required? Be clear to yourself and your future employer. Don't try and separate the two.
